Medical Records Request

According to the Health Insurance Portability and Accountability Act (HIPAA), copies of outpatient medical records can only be transferred upon written request. If you would like copies of your Columbia University Medical Center records sent to a physician outside of the ColumbiaDoctors Neurology practice, please complete our Medical Request Authorization Form English version/Spanish version and fax it to 212-342-4536, or mail it to:

For questions pertaining to medical records, please call 212-342-4517. In accordance with HIPAA guidelines, there is a 75 cent per page fee for copies of medical records.
